Before submitting your application, please ensure you meet the following requirements:

Complete School Application – A $50 non-refundable registration fee is required.

Entrance Test – All applicants must take and pass an entrance test.

Educational Documents – Submit a copy of your GED, high school diploma, certification, or higher education degree.

Valid Identification – Provide a valid ID or passport.

Social Security Card or ITIN – A copy is required for verification.

Physical Exam – Must have been completed within the last 12 months.

Fingerprint & Background Check – You will be registered with the Illinois Department of Public Health (IDPH) for this process.

CPR Certification – CPR training will be provided in the program, but a valid CPR card is required.

COVID-19 Vaccine Proof – A copy of your COVID-19 vaccination record must be submitted.
